California residents who want to know what Personal Information (as defined in the CCPA) we maintain about them may submit a Request to Know using the form above or by calling us at 1-855-950-2595 during business hours.

We will confirm receipt of your request within 10 days and fulfill your request within 45 days, or up to 90 days if we notify you of the reason for the delay. You will be required to verify your identity before we can process your request. If we cannot verify your identity, or if the data is too sensitive to share, we may not be able to provide the information. You may submit up to two Requests to Know per calendar year.

VIRGINIA RESIDENTS — To the extent required by the Virginia Consumer Data Protection Act (VCDPA):

Virginia residents who wish to know what Personal Information (as defined in the VCDPA) we maintain about them may submit a Request to Know using the form above or by calling 1-855-950-2595 during business hours. We will confirm your request within 10 days and respond within 45 days, or up to 90 days if an extension is needed. If we cannot verify your identity, or the data is too sensitive to disclose, we may not be able to provide the information.

Your request will not be completed until we can verify your identity, and if applicable, the identity and authority of any authorized representative making a request on someone else’s behalf. Requests that cannot be verified may be denied.
